- Hours: Mon–Fri 9–12; Tue/Thu also 2–5pm.
- We accept all insurance except UnitedHealth.
- Office: 625 N 5th Ave Ste 4, Sequim, WA 98382-5062 — across from the YMCA of Sequim (610 N 5th Ave).
